Ocean Mind’s referral process is simple, secure and confidential. Referrals must be completed by a support worker, psychologist, mental health worker, school support or other professional supporting the participant.

Complete the form below, which will be sent to the project coordinator.

Once received, the Project Coordinator will contact you to discuss the referral and provide details of the next program. The participant will only be contacted by us directly once a program placement is available.

We will contact you again for feedback about the impact of the course on your client. Any data sent through the form will not be used for marketing or fundraising purpose.
